Sliding Fee Scale

What is the Sliding Fee Discount Program?

The Sliding Fee Discount Program is a voluntary discount program offered by Gateway Therapies, LLC. It allows clients to pay reduced fees for services based on household size and income.

This program is not a government program. It is offered by our practice to help make services more accessible.

Who Can Apply? 

​

• Anyone may apply.

• Must provide household income information to qualify for discounted fees.

• If income information is not provided, full fees apply.
   

How is eligibility determined?

Eligibility is based on:

1. Household size (see “Family/Household Definition” below)

2. Combined gross household income from the last 30 days

 

Clients must provide documentation of income for all household members. If documentation is not provided or eligibility requirements are not met, the full fee applies.

How to Apply
 

1. Notify our staff that you wish to apply for the program.

2. Schedule an appointment with our business office manager to complete the Sliding Scale Eligibility Form and review documentation.
    

Acceptable Proof of Income
 

Income for all household members can be documented with any of the following:

• Prior-year tax return or W-2

• Recent pay stubs (last 4 weekly or 2 biweekly periods)

• Unemployment benefits statements/checks

• Social Security or pension statements

• Alimony or child support documentation

• Employer attestation of income

• Bank statements showing direct deposits
   

Note: Only household members as defined below are counted; roommates or non-family members are excluded.

 

Family/Household Definition
 

Included in Household:

• Parent(s) or legal guardians

• Spouses or significant others

• Children or other dependents supported by the household

• Elderly parents living at home without independent income

• Adult disabled dependents living at home who rely on household income for support
   

Not Included:

• Roommates or non-family members

• Unborn children

• Adult children over 18 not in school with their own income

 

Dependent Children:

• Under 18 living at home: income not counted

• Ages 18–21, in school and living at home: income not counted
   

Adult Disabled Dependents:

• Adults with disabilities who live in the household and rely on household income count as dependents

• Any independent income they may have will be considered separately

Program Duration 

• Discounts begin the day your application is approved

• Discounts are valid for 12 months

• Reapplication is required each year or sooner if household size or income changes
   

Fees

Approved discounts are based on:

• Household size

• Household income

• Service type
   

Clients receive a summary of their discounted rate at the time of approval. Payment is due at the time of service.
